When a hospital discharge is coming

The discharge planner will ask for a decision in a day or two. Here is how to use that time.

Quick answer

Ask the case manager three things the same day: whether your parent is an inpatient or under observation, what level of care the team recommends after discharge, and which facilities or agencies have accepted the referral, then check each one's AHCA and CMS record before agreeing.

What should I do first when a Sarasota hospital plans to discharge my parent?

Ask the case manager three things the same day: whether your parent is an inpatient or under observation, what level of care the team recommends after discharge, and which facilities or agencies have accepted the referral, then check each one's AHCA and CMS record before agreeing.

The acute-care hospitals in the two counties are listed on CMS's hospital data: Sarasota Memorial (Sarasota and Venice campuses), HCA Florida Sarasota Doctors, Manatee Memorial, HCA Florida Blake, Lakewood Ranch Medical Center and HCA Florida Englewood. Their case managers work from the same short lists every day; you may choose any Medicare-certified provider that accepts your parent.

Will Medicare pay for rehab after the hospital?

Medicare Part A pays for skilled nursing facility care only after a qualifying inpatient stay of at least three days, and only while daily skilled care is needed; observation days do not count, and Medicare Advantage plans may require prior authorization and network facilities.

If your parent was under observation, ask whether the status can be reviewed, and ask about inpatient rehabilitation hospitals or home health as alternatives.

How do I choose a rehab nursing home quickly?

Narrow the hospital's list to homes with a bed and your insurance, then compare CMS's overall and staffing ratings, total nurse staffing hours, the latest inspection's deficiency count and the distance from family; our nursing home pages show each figure beside the two-county median.

AHCA licenses 41 nursing homes in Sarasota and Manatee counties with 4,598 beds; CMS rates 41 of them, 16 with four or five stars overall (Care Compare data as of October 1, 2026).

Can my parent go straight from the hospital to assisted living?

Yes, if your parent meets the ALF's admission criteria: able to transfer with help, not bedridden, and not needing 24-hour nursing supervision or care such as wound vacuums or IV therapy that a standard ALF cannot provide; an ALF needs a physician's examination before admission.

An LNS or ECC facility, or a standard ALF plus a home health agency, can bridge the first weeks. Ask the hospital to send orders for home health to the ALF address.

What if my parent wants to go home?

Ask the hospital to order Medicare home health if your parent is homebound and needs skilled care, line up help for the first nights, and check that medications, equipment and follow-up appointments are arranged before leaving.

Paid help at Florida's 2025 median runs about $32 an hour; even a few nights can make the first week safe.
