Of 128 licensed ALFs in Sarasota and Manatee, 30 hold limited nursing services, 12 extended congregate care and 6 limited mental health licenses. ECC is the one that most reduces the risk of a second move.
What are Florida's specialty ALF licenses?
Every Florida assisted living facility starts with a standard license under s. 429.07, Florida Statutes. On top of it, AHCA can approve three specialty licenses. Limited nursing services (LNS) lets the facility's own nurses perform a defined list of nursing tasks for residents who still meet standard admission criteria. Extended congregate care (ECC) lets the facility provide nursing and supportive services beyond the standard scope so residents can stay as they become more impaired. Limited mental health (LMH) is required when a facility serves residents with a serious mental illness who receive optional state supplementation.
None of them is a quality grade. Each describes what may be done in the building, and each adds AHCA monitoring visits and fees.
How common are they locally?
AHCA's profiles show the specialty licenses for every facility. By mailing address:
| City or community | Licensed ALFs | LNS | ECC | LMH | Standard only |
|---|---|---|---|---|---|
| Sarasota | 58 | 14 | 9 | 1 | 35 |
| Bradenton | 31 | 6 | 0 | 3 | 22 |
| Venice | 20 | 5 | 1 | 0 | 14 |
| Lakewood Ranch | 6 | 3 | 0 | 0 | 3 |
| North Port | 5 | 0 | 1 | 1 | 3 |
| Palmetto | 2 | 0 | 0 | 1 | 1 |
| Englewood | 4 | 1 | 0 | 0 | 3 |
| Nokomis | 2 | 1 | 1 | 0 | 1 |
Most facilities hold a standard license only. ECC is the rarest of the nursing-related licenses, with 12 facilities, because a facility must have been licensed for two years without serious sanctions to get it, and the first ECC license is provisional for up to six months.
Why does ECC matter most for families?
The hardest moment in assisted living is often a forced move: a resident's needs grow past what the facility's license allows, and the family has 45 days to find a nursing home or a different ALF. An ECC facility sets its own continued-residency criteria within AHCA's guidelines and can keep residents longer, though still not anyone who needs 24-hour nursing supervision.
The ECC facilities in the two counties are: Neurorestorative Florida (Sarasota), Neurorestorative Florida (Sarasota), Angels Senior Living at Sarasota (Sarasota), Ashton Place Corp (Sarasota), Bayshore Guest Home (Nokomis), Convivial Jacaranda Trace, LLC (Venice), Discovery Commons South Biscayne (North Port), Neurointernational Healthcare, LLC (Sarasota), Sarabella Senior Living (Sarasota), Sarasota Assisted Living LLC (Sarasota), The Barclay at Sarasota (Sarasota), The Highlands at the Glenridge on Palmer Ranch (Sarasota). Ask whether ECC covers the whole building or only some apartments, and whether moving into ECC services changes the monthly rate.
What should you ask about LNS?
Limited nursing services means the facility's nurses can do more on site, for example certain wound care or catheter care, instead of the resident contracting with an outside home health agency. Ask which tasks the facility actually performs under LNS, how many nurse hours are scheduled on each shift, and whether an LNS nurse is in the building at night.
A registered nurse from AHCA visits LNS facilities at least annually to monitor residents receiving those services, and ECC facilities twice a year, unless the facility qualifies for a waiver based on a clean record.
How do specialty licenses show up on inspections?
AHCA inspectors check the specialty services along with the standard rules, and an RN from AHCA or a contracted agency visits ECC and LNS facilities to monitor residents receiving those services. Facilities with ECC must keep written progress reports on each resident who receives nursing services, describing the type, amount, duration and outcome of care.
Specialty licenses can also be lost: AHCA may deny or revoke an ECC license if the facility no longer meets the criteria, which include having no class I or class II violations in the prior two years.
What does LMH mean for other residents?
A limited mental health license means the facility serves at least one resident with a serious mental illness who receives Optional State Supplementation, under a community living support plan coordinated with a mental health provider. It does not change the building's admission rules for other residents.
Families sometimes worry when they see the license; the better question is how the facility staffs and supports every resident, which the inspection history and a visit answer.
How do I filter for specialty licenses?
On the assisted living roster, use the license-type filter: options such as Standard + LNS or Standard + ECC show only the facilities holding those licenses, and the city filter narrows them further. Every facility page licensed for 40 or more residents explains its specialty licenses in plain language and lists questions to ask about them.
Frequently asked questions
Which Florida ALF license lets residents age in place?
Extended congregate care (ECC), which lets a facility keep residents whose needs would otherwise require a move.
Is an LNS facility a nursing home?
No; it is an assisted living facility whose own nurses may perform limited nursing tasks.
Where can I see a facility's specialty licenses?
On its AHCA FloridaHealthFinder profile, reproduced on our facility pages and roster.
Do ECC facilities cost more?
Usually; residents receiving ECC services get more care, but there is no published Florida ECC price, so ask how the fee schedule changes.
Can a facility lose its ECC license?
Yes; AHCA can deny or revoke it when the facility no longer meets the criteria, including a record free of serious violations.
